Ash Grove Healthcare Facility earns 5-star CMS rating
Ash Grove Healthcare Facility has earned a five-star rating from the Centers for Medicare and Medicaid Services (CMS). This is the highest rating available through the CMS Five-Star Quality Rating System.
Ash Grove Healthcare Facility is owned and operated by Citizens Memorial Health Care Foundation in Bolivar. The facility provides skilled nursing, memory care, and inpatient and outpatient rehabilitation services. It also offers duplex-style, two-bedroom and two-bathroom independent living apartments.
The Five-Star Quality Rating System helps families and caregivers compare nursing homes. Each facility receives an overall rating, as well as separate ratings for health inspections, staffing and quality measures. A five-star rating means a facility is “much above average” in overall quality, according to CMS.
“We are incredibly proud of the Ash Grove team for reaching this milestone,” said Tim Francka, administrative director of long-term care at CMH. “It takes everyone in the facility to make this happen, and it takes a team dedicated to providing the best quality care while meeting or exceeding regulatory standards.”
Francka credited the facility’s leadership team for ensuring staff have the resources and training needed to care for residents and remain aligned with the CMH mission.
He added that while the CMS star ratings are helpful, families should also visit care centers in person.
“CMS intends for these ratings to be used along with other sources of information,” Francka said. “The best way to evaluate a long-term care facility is to visit and see the care and compassion being delivered.”
CMH operates six long-term care facilities and one residential care center in Cedar, Dallas, Greene and Polk counties. Ash Grove Health Care Facility joins Parkview Health Care Facility, another CMH long-term care facility in Bolivar, in earning a five-star rating. Three other CMH facilities earned four-star ratings.
Joshua Solomon, LNHA, is the administrator of Ash Grove Healthcare Facility. He will be stepping down to pursue a career in nursing. Alecia Robinson, LNHA, will assume the administrator position on June 9.
