  • Bequest - A bequest is one of the easiest gifts to make. With the help of an attorney, you can include language in your will or trust specifying a gift to be made to Citizens Memorial Health Care Foundation as part of your estate plan, or you can make a bequest using a beneficiary designation form.
  • IRA Rollover - If you are 70½ or older, an IRA charitable rollover is a way you can help continue our work and benefit this year. Contact your IRA plan administrator to make a gift from your IRA to us and for more information.
  • Beneficiary Designation Gifts - A beneficiary designation gift is a simple and affordable way to make a gift to support Citizens Memorial Health Care Foundation. You can designate us as a beneficiary of a retirement, investment or bank account or your life insurance policy. To make your gift, contact the person who helps you with your account or insurance policy, such as your broker, banker or insurance agent.
  • Charitable Gift Annuity - The charitable gift annuity program is a strategic donation method wherein you make a contribution to Citizens Memorial Health Care Foundation and, in return, receive a stream of fixed income for the rest of your life, or that of a designated beneficiary. In addition to the income you receive, you will also be eligible for a variety of tax breaks depending on the type of donation you made. For more information contact your wealth advisor.
  • Charitable Remainder Trust - With this trust type, you choose to receive an income from the distribution of the non-income-producing assets you placed into the trust first. You'll also receive a charitable donation tax deduction based on the present value of the remainder of the assets earmarked for the charity. CMH Heroes for Hospice 5K/10K Run presented by Craig Lehman Shelter Insurance

CMH Heroes for Hospice 5KBe a hero! The Citizens Memorial Health Care Foundation’s “Heroes for Hospice” 5K/10K Run presented by Craig Lehman Shelter Insurance is the first Saturday in May. Both courses begin in front of the CMH Community Rooms and conclude with a loop around Dunnegan Park. The race begins at 8:30 a.m. and is electronically timed for accuracy. Medals are awarded to overall male and female in both races and to first place male and female winners in each age division. Participants dressed as their favorite superhero is encouraged! Walkers, strollers and pets are welcome!

CMH Hospice caregivers strive to help patients remain as comfortable as possible at home, or in a homelike setting, with family and friends. The program also provides support to the family after a death of their loved one through a bereavement program.  Proceeds from the race will provide donations in our area to ensure that physical, emotional, spiritual and bereavement care is given to all terminally ill patients and their families.

Medical Excellence Golf Classic

Medical Excellence Golf Classic

The Citizens Memorial Health Care Foundation Medical Excellence Golf Classic is held the first Thursday in June.  The classic is an 18-hole, four-person scramble at Silo Ridge Golf and Country Club in Bolivar. All monies raised goes to the Medical Excellence Scholarship.

The scholarship provides financial assistance to individuals from the CMH service area who are working toward careers in healthcare. Upon completion of their education, Medical Excellence recipients return to work at CMH.

The tournament will feature two shotgun starts at 7:30 a.m. and 1 p.m. Each player will receive a gift bag, grab and go breakfast, lunch, snacks, range balls and complimentary Coca-Cola products..  Green fees and cart rentals are included in the entry fee.  Each player also will be entered into course contests and mulligans are available at $40 per team.  Prizes for first, second, third and fourth will be awarded in four flights.  There is a maximum of 55 teams and spots will be filled on a first-come, first-serve basis.

Hospice Lights of Remembrance

Hospice Lights of RemembranceCitizens Memorial Hospice holds the Lights of Remembrance “Cheer-city “ Open House and Fundraiser in November.  The purpose of Lights of Remembrance is to honor the life of a loved one, support the Hospice philosophy of living life fully, comfortably and dying with dignity and to involve the community in the support of Hospice care. Individuals can make donations of $10 to $500 per light in honor of family and friends, or in memory of loved ones.

The lights decorating the tree are dedicated to those people who are being honored or memorialized. Giving to the Lights of Remembrance program provides donations in our area to ensure that physical, emotional, spiritual and bereavement care is given to all terminally ill patients and their families.
