Community Springs opened a 15-bed high-functioning memory care unit on Feb. 1, 2024. The unit is a caring place for those with Alzheimer's or other memory issues. Their needs are unique, and our team is here to give personalized support in a warm environment.

Community Springs offers rehabilitation services. Speech, physical and occupational therapy are provided by full-time, in-house therapists.
Memory Care Unit
The memory care unit is a caring place for those with Alzheimer's or other memory issues. Their needs are unique, and our team is here to give personalized support in a warm environment.
Our staff is trained in dementia care, giving residents kind and understanding help with daily tasks and activities. The unit is a safe and cozy home with features that ensure everyone feels secure. Our team is available 24/7, providing continuous support for both residents and their families.
Nutritional Services
The nutritional services staff at Community Springs works with a registered dietitian to provide residents with balanced and nutritional meals.
Activities
Planning and research go into the daily planned activities for residents at Community Springs. Residents enjoy a variety of activities to foster a sense of purpose. Activities include luncheon outings, bingo, musical entertainment, arts and crafts, hands-on gardening and so much more.

Community Springs Senior Living Community allows seniors to remain independent in a home of their own. Apartments are two-bedroom, two-bathroom units. The apartments are located on the same campus as Community Springs, near healthcare professionals.

Meet the Leadership
Vautrin Cadle
Administrator
He oversees all functions of the healthcare facility. He follows the overall policies, procedures and regulations established by the governing board and local, state, and federal governments.
Brittany Hardman, R.N.
Director of Nursing
Responsible for resident care, coordinates visits of physicians and care consultants, and is involved with each resident care plan. She is also responsible for hiring quality staff.
Rebecca Caylor, R.N.
Assistant Director of Nursing
Plans, organizes, implements, evaluates and coordinates the development, education, orientation and scheduling of the nursing staff in accordance with Federal and State regulations and quality care needs.
Scott Stone
Director of Nutritional Services
Scott has dedicated his career to the food industry since 2005. He is responsible for all aspects of nutritional services at the facility.
Jason Hardman
Director of Plant Facilities
Jason is responsible for maintaining the buildings and grounds. He coordinates new construction and renovations and monitors resident and employee safety and security.
Rita Kaye Miller
Director of Environmental Services
Assures a clean, sanitary, and uncluttered environment for residents, families, and staff. Rita conducts regular inspections to ensure quality control.
Tina Pellegrin Tryon
Director of Social Services
Assists with discharge planning, advanced healthcare directives, crisis interventions, resident/family concerns and compliments. Tina assists with transportation and ensures rights and dignities are upheld.
Kathy Crusha
Activity Director
Kathy has been the Director of Activities since 2019. She is responsible for planning each individual’s activity needs and scheduling activities and events. Kathy also organizes and directs our volunteer program.
Missie Steward
Business Office Manager
Assists in the information and direction process to our residents, families and visitors, and receives and records payments.
Susan Stone, LPN
Resident Care Coordinator, MDS
Responsible for completing Medicare assessments for state and federal government. Sue facilitates care plans and ensures smooth transitions from the hospital to skilled rehab.
Ariel Swopes
Admission Coordinator
Ariel coordinates admissions for new residents to ensure a smooth transition to Community Springs.
Mary Cochran, B.S., RDN, L.D.
Dietitian
Provides nutrition assessment for each resident. Mary monitors food and fluid intake, weights and skin integrity and is responsible for providing education to Nutritional Services.
