The Missouri State High School Athletics Association (MSHSAA) requires the forms prior to the first practice. CMH's board-certified physicians, nurse practitioners and physician assistants administer the physicals.

CMH Clinics recommend that your student-athlete complete an annual wellness examination and have the sports physical included in the appointment. During sports physical days, your athlete can receive a free sports physical at your family's clinic.
