Dian Hofmeister receives May 2026 CMH DAISY Award
Dian Hofmeister, RN, is the recipient of the May 2026 CMH DAISY Award for Extraordinary Nurses. Hofmeister is a registered nurse with Citizens Memorial Hospital’s Home Care Services.
Three patients’ family members nominated Hofmeister, describing the compassion, kindness and comfort she provided during the final days of their loved ones’ lives.
Each nomination shared a similar message: Hofmeister cared not only for her patients but also for the families walking beside them. The families praised her for answering questions at any hour, listening patiently, offering reassurance and helping them navigate the hospice journey with dignity and compassion.
One family member described Hofmeister as “an angel” during one of the hardest times in their life.
“Death is scary. Death is messy. Dian made it less so for all of us,” the nominator wrote.
Another nominator shared that Hofmeister cared for several generations of the family over the years, describing her as “very caring, empathetic and compassionate to the very end.”
Although Hofmeister cared for many patients, the nominator wrote, “the way she shows up, carries herself and cares, you’d think she was part of the family.”
A third family expressed gratitude for Hofmeister’s patience, skill and kindness.
“The kindness that Dian has shown to us touches us deeply,” the family wrote. They also praised Hofmeister for keeping a promise to make their loved one comfortable and said they were “very fortunate to have Dian as our RN.”
Hofmeister’s nominations reflect the lasting impact she has on patients and families by providing comfort, dignity and peace during life’s most difficult moments.
About the DAISY Award
DAISY is an acronym for Diseases Attacking the Immune System. The DAISY Foundation established the award in memory of J. Patrick Barnes, who died at age 33 of an autoimmune disease. The international program recognizes the extraordinary and compassionate care nurses provide every day.
CMH began participating in the DAISY Award program in January 2019. Each month, a nurse from Citizens Memorial Hospital or Citizens Memorial Health Care Foundation is selected for the award.
