CMH Mobile Integrated Healthcare care teams bring health and wellness support to your home. They work with primary care providers to support clinical, social, emotional and behavioral needs. MIH helps ensure patients who need it most have access to quality care.

Who is Eligible for MIH Services

MIH helps patients who need extra healthcare support but face access barriers. It extends primary care and is led by physicians. MIH can connect patients with a primary care provider for ongoing needs. It enhances patient safety, health outcomes, quality of life, and satisfaction.

MIH Behavioral Health Services

The MIH program offers in-home or virtual visits with a licensed Behavioral Health Provider for patients facing barriers to traditional care. An experienced MIH care team member gives additional support. Whether you’re navigating anxiety, depression or trauma, or need extra support managing mental health, we bring the help to you.

Counseling Sessions

  • A CMH MIH care team member will be there to help with wellness needs.
  • Support for anxiety, depression, grief, trauma and stress.
  • Coordination of care with your primary care and specialty providers.
  • Help with medication management in collaboration with prescribers.
  • Referrals to community resources, case management and additional services.
  • Emotional wellness screenings and risk assessments.

MIH OB Support and Lactation Consultations

You can get help from a skilled OB nurse and a board-certified lactation consultant. We provide expert support for high-risk moms and newborns who can't access a clinic.

OB Support and Lactation Services

  • In-home maternal health assessments by an OB nurse.
  • Postpartum education and monitoring.
