Diabetes Education

CMH Diabetes Education provides individualized nutrition counseling and diabetes self-management education for people with Type 1, Type 2 and gestational diabetes. Services are provided by Ladonna Webb, RDN, LD, CDCES, a registered dietitian and certified diabetes care and education specialist.

Individualized Plans

Education is tailored to each person's needs and may include:

  • Healthy eating
  • Physical activity
  • Blood sugar monitoring
  • Medications
  • Problem-solving skills
  • Healthy coping strategies
  • Risk reduction

Personalized support

Managing diabetes affects many parts of daily life. Our program helps patients develop practical skills and confidence to make informed decisions at home, at work, while traveling and during special occasions.

Physician referral

A physician referral is required for diabetes education services. Talk with your primary care provider or specialist about a referral.

Insurance

Medicare and most private insurance plans cover diabetes education. Medicaid coverage requires prior authorization.
