Medical Student Elective Rotations
CMH welcomes medical students for elective rotations. This page provides vital information for students and medical school staff.
General student guidelines include:
- You must be currently enrolled in an accredited M.D., D.O., NP, PA, Pharmacist or CRNA program.
- CMH must have an affiliation agreement with your institution on file.
- Rotation dates are flexible to fit your specific needs. Submit your application at least six months before your needed start date.

How to Apply for a Rotation
You must apply for an elective rotation. Elective placement is made based on a combination of candidate qualifications, physician availability and timeliness of application. Required items are:
- Completed application. (Contact Sara Bible at sara.bible@citizensmemorial.com for the application.) This form is completed for each rotation you request to complete at CMH.
- Signed Attestation Agreement (To be signed by the student AND the physician/practitioner that both agree to complete the rotation).
- Letter of good standing from your medical school.
- Current certificate(s) or declaration(s) of insurance.
- Copy of Criminal Background Check (No more than one year old).
- Proof of completed fit test and supply of personal N95 masks. CMH does not offer fit testing. (If proof is not provided, the student will be excluded from airborne precautions and COVID-19 patients.)
- Proof of a TB test that is no more than a year old.
- Copy of BLS card.

Vaccination Policies
Influenza Vaccination Policy: CMH requires all employees to be vaccinated for influenza annually. Each year, for rotations between Oct. 1 and March 31, all students must show proof of influenza vaccination. Certain documented medical exemptions are allowed.
COVID-19 Vaccination Policy: CMH requires all employees to have proof COVID-19 vaccination since 8/22/24 or submit a signed declination.
