Sports Physicals

Students participating in middle school and high school athletics must complete a pre-participation physical evaluation form before starting a camp or practice. The Missouri State High School Athletics Association (MSHSAA) requires the forms prior to the first practice. CMH's board-certified physicians, nurse practitioners and physician assistants administer the physicals.

Willard Medical Center and Walk-In Clinic recommends that your student-athlete complete an annual wellness examination and have the sports physical included in the appointment. During sports physical days in July and August, your athlete can receive a free or discounted sports physical at your family's clinic.
