Services Provided

At Pleasant Hope Family Medical Center, we see patients of all ages for a variety of illnesses, injuries and ailments. Our medical staff also provides complete family care, well-baby exams, well-woman exams, Medicare wellness visits, immunizations, behavioral health and same-day appointments. Pleasant Hope Family Medical Center is accepting new patients.

Sports Physicals

Students participating in middle school and high school athletics must complete a pre-participation physical evaluation form before starting a camp or practice. The Missouri State High School Athletics Association (MSHSAA) requires the forms prior to the first practice. CMH's board-certified physicians, nurse practitioners and physician assistants administer the physicals.

CMH Clinics recommend that your student-athlete complete an annual wellness examination and have the sports physical included in the appointment. During summer sports physical days, your athlete can receive a free sports physical at Pleasant Hope Family Medical Center.
