Medication Management Services

Keeping up with your medications can be confusing. We’re here to make it easier. At the CMH Medication Management Clinic, you’ll meet with one-on-one with a pharmacist who can help you understand your medicines and make sure they’re working for you. We’ll review your medications, answer your questions and coordinate with your doctor to make sure your therapy is safe and effective.

  • Comprehensive medication reviews: We look at all your medicines to make sure they are safe and working well together.
  • Patient education appointments: Learn what your medicines do and how to take them correctly.
  • Medication support: If you take several medicines, we help simplify and organize them.
  • Dosing adjustments: We review your medicines and suggest dose changes to improve results.
  • Medication therapy recommendations: We give guidance to help your therapy work better for you.
  • Guideline-based therapy and dose optimization: We make sure your medicine plan follows best practices for your condition.
  • Complex interactions: We check for possible problems between your medicines and other health conditions.
  • Bridge or taper schedules: We create plans to safely start, adjust or stop medicines.
  • Side effect management: We help identify and manage any unwanted side effects from your medication regimen.
  • Medication cost concerns: We look for ways to make medicines more affordable or make recommendations for cheaper alternatives (if available).
  • Other drug questions: Ask us anything about your medications.
  • Closing care gaps: We work with your care team to make sure all recommendations for treatments and preventive care services are up to date.
  • Self-injectable medication training and education.
  • Continuous Glucose Monitor training and education.
