The CMH Optical Shop is conveniently located next to the Eye Center and has a display of frames, lenses and sunglasses for sale.

Records also can be requested by completing and returning the Release of Information Authorization Form PDF via mail or fax. A medical record copying fee will apply. Please allow at least 3-5 business days to fill your request.
