COVID-19 Testing
CMH has a COVID-19 hotline number. Patients and community members with coronavirus-related questions, including having possible symptoms of COVID-19, are asked to call their primary care provider.
Testing Locations
COVID-19 testing is available at CMH's primary care clinics and at Stephens Pharmacy in Bolivar. Stephens Pharmacy is also a test-to-treat location and can fill a prescription for qualified patients.
Virtual Visits
CMH Virtual Visits are an option for patients with non-emergency illnesses. CMH offers access to health care through the convenience of virtual visits in the comfort of your home, which minimizes the potential of exposure to others. With a virtual visit, CMH providers can diagnose and treat patients for COVID-19 related symptoms and also common medical conditions like sore throat, sinus problems, stomachaches, nausea and vomiting, fever, skin problems and more – all from a secure video chat on a mobile device or desktop computer. Clinic Visits
If you have symptoms of COVID-19 and you have an appointment to see your primary care provider, please call your clinic ahead of time so we can prepare for your arrival.
CMH Patients and Employees
- CMH surgical services patients may be tested for COVID before a surgical procedure if symptomatic. Patients should report to CMH Outpatient Surgery Center registration located in the Kerry & Synda Douglas Medical Center, 1155 W. Parkview St., Bolivar.
- CMH patients who need COVID-19 testing due to an exposure or symptoms or need to be tested for any other reason should contact their primary care providers for testing services at their CMH clinics.
- CMH employees who need COVID-19 testing due to an exposure will receive information from the CMH employee health coordinator and/or infection prevention coordinator.
Non-CMH Patients and Community
- Non-CMH surgery patients will be directed by their primary care providers for COVID-19 testing services.
- Fresenius Kidney Care patients will be directed by their medical providers for testing services.
