Visitation rules for visitors and staff at Citizens Memorial Health Care Foundation long-term care facilities.

Visitors

Please self-assess and postpone visit if you have or recently had any of the following:

• A positive influenza or COVID test in the last 10 days
• The following symptoms related to respiratory infections such as:
  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
• Close contact with someone with influenza or COVID

If you must visit, please wear a mask at all times while in the facility.

Staff

Please self-assess and notify your supervisor immediately if you have or recently had any of the following:

• A positive influenza or COVID test in the last 10 days
• The following symptoms related to respiratory infections such as:
  • Fever or chills
  • Cough

  • Fatigue
