Rural Health Clinic Sliding Fee Discount Program

We believe every patient deserves high-quality, compassionate care. We are committed to helping patients access the services they need, regardless of their ability to pay.

Our Sliding Fee Discount Program helps reduce the cost of care at our rural health clinics (primary care), including the CMH OB/GYN and Pediatric Clinic. This program is supported by federal grant funding, which allows us to extend affordable care to more patients in our community.

How to Apply

Complete the Sliding Fee Scale Application (PDF) and submit it, along with the required documentation, to CMH Customer Service.
