Patient/Family Advisory Council

CMH values feedback from patients and their families. Members of the Patient/Family Advisory Council (PFAC) help CMH improve the overall patient experience, safety and outcomes. Their voices offer the view of how it feels to be a patient. They provide valuable feedback for identifying ways CMH can make sustainable improvements. Patient and family perspectives promote a culture of patient and family-centered care.

PFAC Member Roles and Responsibilities

  • Serve as an advisory resource to administration and staff.
  • Advocate for education and clinical programs to improve the quality and accessibility of care.
  • Assist in the development and review of educational materials.
  • Respectfully listen and tactfully discuss ideas, issues and concerns.
  • Respect the privacy and confidentiality of council members, their families and all staff.
  • Actively participate with expectations of time investment outside of the meetings.

PFAC Member Requirements

  • Be a patient or a family member of a patient who receives services at CMH.
  • Commit to serving on the CMH PFAC for a minimum of one year. Terms begin when the application is approved.
  • Attend at least 50 percent of the PFAC meetings within a 12-month period. (100% attendance is encouraged.)
