Citizens Memorial accepts most major insurance plans, but you should contact your insurance company directly to confirm eligibility and benefits. Insurance coverage may vary by type of service: Clinic, hospital, long-term care, home health, hospice, etc.

Patients are responsible for co-pays, co-insurance or deductibles. CMH accepts cash, check or credit card. You may be eligible for financial assistance from CMH if you do not have insurance.
