CMH Senior Fitness Center Enrollment Form

Membership at the CMH Senior Fitness Center is free and open to individuals in Polk, Greene, Dallas, Dade, Hickory, Cedar and St. Clair counties. To be eligible, you must be 55 years of age or older.

Complete the online form below to start the CMH Fitness Center enrollment process. As part of the enrollment form, you'll complete a Physical Activity Readiness Questionnaire to determine if you require a Physician's Clearance.

After you submit the form, we'll contact you to go over your enrollment information. If you require a Physician's Clearance, we'll send the form to your physician.

Before your enrollment is approved, you'll also sign a waiver of liability form and complete a 30-minute orientation.
