Community Health Needs Assessment

Citizens Memorial Hospital prepares a Community Health Needs Assessment (CHNA) every three years beginning in 2013. The CHNA:

Describes the Hospital and its services.
Provides an overview of the Patient Protection and Affordable Care Act and the impact of government legislation on Hospital operations.
Summarizes the identified community health needs and the Hospital's plans to help meet those needs.

As a tax-exempt hospital district, the report is prepared in compliance with Section 501(r)(3) of the Internal Revenue Code.
