Rehabilitation Services
CMHCF offers rehabilitation services. Speech, physical and occupational therapy are provided by full-time, in-house therapists.
Nutritional Services
A registered dietitian works with the nutritional services staff at CMHCF. Together, they provide residents with meals that are balanced and nutritional.
Activities
Planning and research go into the daily planned activities for residents at CMHCF. Residents enjoy a variety of activities, including pet therapy, bingo, kids club, socials, annual beauty pageant, holiday-themed activities and more.

Independent Living
Located near CMHCF, Butterfield Apartments offers a community-style apartment complex in a one-bedroom, one-bathroom floor plan. Units are fitted with appliances including a stove, refrigerator and freezer, garbage disposal, washer and dryer. Other features include paid utilities, pull cord and pendant alarms, network smoke detectors, and central heat and air. CMH provides a 24-hour emergency call service, lawn care, landscaping and routine maintenance. The apartments are located on the same campus as Citizens Memorial Healthcare Facility. Meet the Leadership
Cindy Cain
Administrator
Cindy promotes maximum quality of life, dignity, socialization, activity, and nutrition for the residents. Her goal is to maintain each resident’s highest possible level of cognitive and physical functioning in a comfortable and protected environment.
Katie Watkins, R.N.
Director of Nursing
Katie is responsible for all aspects of resident care and supervises all of the nursing staff.
Ashley Wood
Activity Director
Coordinates the activity program. Ashlee provides residents with various opportunities to meet physical, social, emotional and spiritual needs through a wide variety of group and independent leisure activities.
Sarah Lake
Director of Social Services
Assists with admissions, advanced healthcare directives, care and discharge planning, resident/family concerns, questions, and compliments.
Dariena Applegate
Director of Environmental Services
Assures the cleanliness of the facility. Dariena has more than 20 years of environmental services experience.
Heather Caldwell
Director of Nutritional Services
Supervises the food preparation and meal services provided to residents, visitors, and staff of the facility.
Forrest Wilhite
Maintenance Supervisor
Responsible for preventative maintenance, appearance and mechanical operations of the facility and makes repairs as needed. Forrest also monitors the safety of the residents and staff.

LTC Rehab Supervisor
Leads the rehabilitation team in order to ensure residents regain maximum potential.
Susan Lenz, SLP
LTC Rehab
Assists the rehabilitation team in order to ensure residents regain maximum potential.
Lindsay Tucker, R.N.
Assistant Director of Nursing/Staff Development Coordinator
Supports the director of nursing in assuring healthcare needs are met. Lindsay is responsible for monitoring and preventing infections and educating staff.
Dayna Mitchem, R.N.
MDS Coordinator
Responsible for all MDS assessments for state and federal government and care plans for the Cribbs/Eastside, and ensures that the needs of the residents are being met.
Mary Cochran, B.S., RDN, L.D.
Dietitian
Provides nutrition assessment for each resident at all seven CMH facilities. Mary monitors food and fluid intake, weights and skin integrity and is responsible for providing education to Nutritional Services.
