Current patients may receive an invitation to join via e-mail, may call the clinic at 326-6021 and provide their e-mail address, or email the clinic at healthforms@bolivarfamilycare.com. Once a member, you can request appointments, view your health information online, even ask a question of your physician’s nurse!
