Citizens Memorial makes scheduling appointments with our providers as easy as possible through a friendly voice or online convenience. See below for information about how to schedule different types of appointments.

Patient Portal for Online Scheduling

Online appointment scheduling requires a CMH Patient Portal account. If you are not a portal user, you can create an account online.

Schedule a Mammogram

A screening mammogram is performed when a patient has no breast complaints or symptoms. You may schedule an appointment for a screening mammogram directly with the CMH Breast Care Center without a physician referral.

Online

Log in to the CMH Patient Portal to schedule an appointment for a screening mammogram.

You may schedule your appointment online if:

  • Your last mammogram was done at CMH.
  • You do not have any current symptoms (discharge, lump, or new pain).
  • You have not had a lumpectomy or been diagnosed with breast cancer within the past two years.
  • You have seen your primary care physician within the past 24 months.
  • You do not have an augmentation.
  • If you do not meet these guidelines, you may request an appointment for a screening mammogram through the patient portal. A mammogram scheduling specialist will call you to schedule your appointment.

Schedule with a Doctor or Advanced Practice Provider

Specialist providers may require a referral from a primary care physician.

Call any CMH clinic during regular business hours to schedule an appointment with a physician, nurse practitioner or physician assistant.

Online

You can log in to the CMH Patient Portal to request an appointment with any provider. A scheduling specialist will call you to schedule an appointment.

Save My Place

Your home is your waiting room with Save My Place, CMH's online check-in service. Use Save My Place at select walk-in clinics for your convenience. Save your place in line from anywhere using your mobile device or computer from the comfort of your home. You can Save Your Place or just walk in to be seen.

Schedule a Virtual Visit

A virtual visit is for non-emergency conditions such as:

  • Sore throat.
  • Sinus problems.
  • Stomach aches.
  • Nausea and vomiting.
  • Fever.
  • Skin problems.
  • Sports injuries.

Some medical conditions cannot be treated through a virtual visit and may require an in-person visit with a provider.
